How to reduce no-shows in a UK dental practice
A practical playbook for fewer missed appointments: confirmations that patients answer, reminders on the right channel, easy rebooking, a working cancellation list and how AI agents run it all.
Why patients miss appointments
Most missed appointments aren't patients deciding not to come. They forget, their plans change and rearranging feels like too much effort, or they meant to call and the line was busy. Some appointments were booked months ago at a recall, and the patient has lost track of them.
That means most no-shows can be prevented by making it easy to confirm, easy to change and hard to forget, and by acting quickly on the slots that do come free.
1. Ask patients to confirm, not just remind them
A reminder tells a patient about an appointment. A confirmation asks them to reply, and their reply tells you something. Patients who confirm are far more likely to attend, and patients who don't reply are the ones to follow up.
Send the confirmation request a day or two before the appointment and make replying effortless: a single tap or word on WhatsApp or SMS. Anyone who hasn't replied by the afternoon gets a call.
2. Use the channel each patient actually reads
Some patients live on WhatsApp, some read texts and some only answer the phone. Record each patient's preferred channel and use it. For appointments booked long in advance, such as recalls and hygiene visits, add a reminder a week before as well as the confirmation request.
3. Make changing an appointment easier than missing it
If the only way to rearrange is to call during opening hours and wait in a queue, some patients will simply not turn up. Let them reply to the reminder to say they can't make it, and offer new times in the same conversation.
An AI agent can do this at any hour: it reads the reply, checks the diary and moves the appointment, so the original slot is freed while there is still time to fill it.
4. Keep a cancellation list that actually works
A cancellation list only helps if someone works through it the moment a slot opens. Keep a list of patients who want an earlier appointment, with the times they can make, and contact them as soon as a slot comes free, in order, until one accepts.
This is exactly the kind of fast, repetitive work an AI agent does well. It can offer the slot to several patients on WhatsApp within minutes and book the first one who accepts.
5. Follow up every missed appointment
When a patient does miss an appointment, contact them the same day. Most will rebook if asked straight away, and you learn why it happened. Make sure your missed-appointment policy is clear when patients book, and apply it consistently, in line with NHS rules for NHS patients.
Measure it
- Failed-to-attend rate. Missed appointments as a share of booked appointments, by appointment type and clinician.
- Confirmation rate. The share of patients who confirm, and how quickly.
- Slots refilled. How many cancelled slots were filled from the cancellation list, and how quickly.
- Rebook rate. How many patients who missed an appointment rebooked within a week.
How Intelinteract runs it
Intelinteract's appointment confirmation agent contacts tomorrow's patients on their preferred channel, records who has confirmed, moves patients who can't make it and offers freed slots to your cancellation list. Everything is written back to your PMS, and your team sees each morning who is coming in.

When should we send appointment reminders?
A confirmation request a day or two before works for most appointments, with an extra reminder about a week ahead for appointments booked long in advance, such as recalls.
Do patients mind reminders on WhatsApp?
Many prefer it. Use each patient's preferred channel, and let them change it or opt out at any time.
Can the AI move an appointment when a patient replies?
Yes. The confirmation agent reads the reply, checks real availability and moves the appointment in your PMS, then offers the freed slot to your cancellation list.
